Payout Requests or Account Transfer Request
If your account and access to this website was cancelled and you have earned rebates that you would still like to have paid out to you, please complete this Online Form and select 'Reward Payout or Missing Transaction Inquiry' as the primary subject. This method of requesting a payout of your remaining rebate balance should only be done if you were not able to successfully login to this club website to access your Reward Account because you either cancelled your membership or it was cancelled because any membership charges, if applicable, were not able to be successfully processed. You will be able to still request a payout and receive any Available Funds within 30 days of any account expiration or cancellation, loaded on to an existing Prepaid Visa Card and mailed to you within 2 weeks of a Payout Request, subject to having these funds available. Any unclaimed funds after 30 days of account expiration or cancellation will be forfeit.

Access to both member websites and all of their benefits are included to all members, thus a member of one program, is a member of both.

BILLING TERMS: Your onetime non-refundable processing fee of $7.95 has been received, and your account has been setup with your assigned Member Number and Pin Number being issued to you and included in your mailed fulfillment materials that is sent to all New Members. Your New Member package includes in-depth program and benefit information, including helpful savings tips and more, for both, My Magazine Rewards and Live Smart Lifestyles.If for any reason you have not received your New Member package by regular U.S. Mail within 7 –10 Days, please feel free to contact Member Benefit Support as indicated above to request a no cost duplicate package be expedited and sent to you with your assigned Member and Pin Numbers with complete instructions to register and login to access your online member websites, or you may also request that your assigned Member and Pin Numbers be given to you over the phone as well so that you can get immediate access to your benefits through both of your included exclusive member websites.

As a new member, you will have received a 14 day free trial, during which you may cancel your membership by calling the above listed Billing Support phone number within the hours of listed operation. Your monthly membership with My Magazine Rewards, that also includes at no added cost, Live Smart Lifestyles, will then process in approximately 15 days for the first month’s payment of $59.90, unless you have canceled already and the monthly transaction will show up on your credit card statement with this Billing Descriptor:

You will then be charged every month for your membership, in the amount of $59.90 unless you choose to cancel your membership which can be done by calling our helpful Billing Support department, no questions asked. You may cancel at any time as you are not required to remain a member, your membership and access to both member programs and benefits are on a month to month basis, allowing you access to all benefits unless you cancel your membership.

FIRST STEP FOR NEW MEMBERS: You must first Activate your online member website access on the My Magazine Rewards website located at www.mymagazinerewards.com by clicking the “Activate” button located on the front, Home page of that site. You will then be prompted to enter your assigned Member Number and Pin Number, and other requested information where indicated, including the choosing of your own Password to complete your registration. You will then be able to Login and enter both of your included member websites, simply by going to www.mymagazinerewards.com and entering your assigned Member Number and the Password you chose during the Activation and Registration process, where indicated in the “Returning Members” area of the front, Home Page of that site to enter the exclusive member benefit area, or by going to www.livesmartlifestyles.com and clicking “Login” located at the top of the main, Home Page of that site, and then entering that same Member Number and Password you chose, in the space provided, enabling to also enter and then use the benefits located on the Live Smart Lifestyles member website. Remember, your same Member Number and Password works on both member websites, to access and use the available benefits.
